Your medical records are confidential and will remain on file under our custodianship according to state and federal statues. Please note, by law we cannot share your medical information without your written consent. Should you need a copy of your medical records you will need to sign an authorization form for their release.

Click on the following link - Submit a Request for Release of Medical Records through this website.  After submitting, open the attached Patient Authorization for Release of Medical Records Form.  Print, complete, sign and date the document and return as directed.
